First Visit/ Initial Consultation
Please arrive to your appointment 10 mintues prior to the scheduled time. Core Collective reception will have a new patient form to complete outlining your details and medical history. Please take the time to read the consent form and cancellation policy form and sign both. 


Together during the initial consultation, we will discuss your major concerns and talk about your medical history. I will then perform an examination to assess for any neurological, orthopaedic and spinal findings. 


Before your first adjustment, I may refer you for spinal X-rays to gather more diagnostic information. We will discuss this together on the day. 

Second visit/ Report of Findings
This visit is your report of findings visit. You will first watch a 5 minute video to explain more about Chiropractic and what to expect from Chiropractic care. I will then explain all of the relevant exam findings and x-ray results (if necessary) and the future care plan, designed specifically for you. 

Cancellations/ Missed Appointments
I understand that we all live busy lives and sometimes cancellations are unavoidable, however I do ask that you respect the needs of the patients on the waitlist. 

If you cancel your appointment with less than 24 hours' notice or do not show for your appointment, a $50 fee will be charged.

Insurance

If your insurance policy covers Chiropractic, an electronic receipt will be emailed to you after each visit for you to make a claim. 

Payments
Payment is due on the day of your appointment. Bank transfer and PayNow via UEN or barcode are the accepted methods. The fees are outlined on the make an appointment page.
